Does anyone remember who made this? It's from a two-parter April Fools swf animation called New Big Change and Real Big Change I downloaded from deviantart over 5 years ago.
The signature forms are really only a formality and the Clinic has a 100% success rate in getting the proper signature. The Female Nurses do all of the procedures as it turned out that the males became squeamish.
